Bulk MVR - Any employer who requests more than 50 MVRs per year.
There are five purposes for requesting MVRs. Click each purpose for a full description.
A separate User ID and Password will be assigned for each purpose requested.
Your completed Bulk MVR package must also include the following
supporting documents:
- A completed Application for Bulk Motor Vechicle Records (MVR)
- The Georgia Technology Authority (GTA) MVR User Agreement
- The Georgia Department of Driver Services (DDS) Bulk MVR Requester Acknowledgement OR Bulk MVR User Acknowledgement
- Insurance companies applying for LRI access on behalf of
their agents, must send in the GTA LRI Agreement.
- A completed signed and notarized Bulk MVR Security Affirmation.
- One of the following documents:
- Business License
- Insurance License
- Occupational License
- SalesTax Exemption Certificate (for Government Agencies)
- A document issued by the Internal Revenue Service (IRS) containing the
Federal Employer Identification Number (FEIN) associated with the business.
- Mail the application package to:
