All applications (New and Recertification) must include the following:
- A completed Application for Bulk Motor Vehicle Records
- A completed and notarized Bulk MVR Security Affirmation
- A document issued by Internal Revenue Service (IRS) containing the Federal Employer Identification Number (FEIN) associated with the business
One of the following:
- The Georgia Department of Drivers Services (DDS) Bulk MVR Requestor Acknowledgement or Bulk MVR User Acknowledgement
- GTA Internet LRI Agreement (for Internet LRI Users)
One of the following (document must be valid and unexpired):
- Business License
- Insurance License
- Professional / Occupational License
- Sales Tax Exemption Certificate (for Government Agencies)
New applications that are for the purposes of insurance, employment, credit, or rental car agency must, in addition to the above, include one of the following:
- GTA MVR User Agreement (for Bulk Users)
- GTA MVR Requestor Agreement (for Bulk Requestors)
